First Visit

We look forward to welcoming you to our office!

Your first visit will consist of an initial clinical examination by Dr. Simi, a discussion of potential need for orthodontic treatment and proper treatment timing. While we can often answer general questions about treatment during the initial examination, specific treatment questions can only be answered after careful analysis of diagnostic records.

Please assist us by providing the following information:
• Medical history form
• HIPAA acknowledgement and authorization forms
• Any panoramic x-ray taken within the past six months
• If you have orthodontic insurance, please bring your dental insurance card.

Diagnostic Records

The first step toward treatment is taking Diagnostic Records. These records are necessary for Dr. Simi to begin to develop a custom treatment plan for each patient.

Diagnostic records usually consist of the following:
• Panoramic x-ray of all the teeth
• Cephalometric x-ray of the skeletal structure of the head
• Intraoral scan of the patient’s teeth
• Facial and intraoral photographs
• Examination by Dr. Simi

Dr. Simi will evaluate the diagnostic records to analyze each patient’s specific needs and develop an in-depth treatment plan, critical to achieving high level results.

Treatment Plan Discussion

During the treatment plan discussion, Dr. Simi will review his clinical diagnosis, treatment options, patient cooperation needed, risks and benefits of treatment and approximate treatment duration.

If the patient is a minor, both the patient and one parent should be available to attend this appointment. We want our patients to leave the treatment plan discussion with a clear understanding of his or her specific needs, what the treatment will consist of and approximately how long it will take.
